Rating scales are to be completed proir to the first appointment.

  1. Childcare rating form (0 – 3 years 11 months)

  2. Kindy/preschool form (4 – 4 years 11 months)

  3. School rating form (5 years+)

  4. Infant parent questionnaire (0 - 2 years 11 months)

  5. Parent questionnaire (3 years+)

  6. Autism Parent rating form (3 years+)
